Your right to a good faith estimate

If you do not have health insurance, or choose not to use it for your care, you have the right to an estimate of expected charges.

Ask for an estimate

You can request a written good faith estimate before scheduling care or when you schedule. It should describe the expected charges for your health care items and services, including related costs. Call 718-634-0005 and let us know that you are uninsured or paying without using insurance.

Ask each provider or facility involved in your care for its estimate. Keep a copy with your records.

When to expect it

  • For care scheduled 3–9 business days ahead, the estimate is due within 1 business day after scheduling.
  • For care scheduled at least 10 business days ahead, it is due within 3 business days after scheduling.
  • If you ask before scheduling, it is due within 3 business days of your request.

If the bill is higher

If a provider or facility bills you at least $400 more than its good faith estimate, you may qualify for the federal patient–provider dispute process. You generally must start within 120 calendar days of the date on the initial bill.
